| Any forms that require information from your pediatrician, including health appraisals, sports or camp forms, should be brought to your child’s appointment. There is no charge for having the doctor complete the forms at the time of the routine exam; however, we charge $10 to complete health forms after your child's scheduled visit. - We ask that routine well visits be scheduled at least two months in advance.
- We are happy to complete all school/health forms during your child’s well visit. Please remember to bring them with you.
